How to sign up for an account with Mobile Midwife
Please click here to access the client portal and set up your electronic health record
If you click on the above link from a phone, you will be able to download the app; otherwise it will take you to the desktop version. I find the desktop version to be a little easier (and less buggy) for filling out the intake questions. However, if you download the app, you will be able to have access to your labs and documents on it forever (we will also send these to you manually).
Please fill out your health history before our first visit; this should take about 20 minutes.
Within the app you will find "messaging" and "chat" options. These are HIPAA-compliant ways to communicate. You should know that regular email and text are not HIPAA-compliant but that if you communicate with me over text and email I will certainly write back. You do not need to use the HIPAA-compliant system; however, I' am required to provide it. You are welcome to use whatever form of communication you feel most comfortable with.
If unable to open the client portal from your computer please follow these steps :
Close all your browsers and try the link again.
Reboot computer and try the link again.
Try a different browser or computer if available.
Try the link as an app instead.
Enable Silverlight plugin :
Safari - Install Silverlight if you haven't already. Then open the menu Preferences->Websites. Then in left hand column under PLUG-INS, check the box next to Silverlight. This will show a dropdown menu on the right-hand side column labeled "When visiting other websites." Click on the dropdown and select "On.".
Internet Explorer - First be sure there is nothing that says “InPrivate” at the top by the web address. If so, restart your browser. Then press the gear icon in the upper right-hand corner and choose “Manage add-ons”. Then choose “All add-ons” under the “Show:” field. Then scroll down on the right and make sure the “Microsoft Silverlight” add-on is “Enabled”. If on Windows XP use Firefox.
Chrome - Google's Chrome browser does not support Silverlight in version 45 and later. We recommend using Safari or Internet Explorer instead.
